Painfull ingrowing toe nail help pls?

always get them, this time its infected again, i got numbing cream from chemist to put on and i managed to pull some of it out but some of the nail is still stuck, doctor is useless always puts me on antibiotics and says put tissue under nail wich never works, usually i have a few drinks (ok a lot) and my husband rips it out but this time its stuck any ideas
